Part D Creditable Coverage Letter
Requirements for Employers
Is your Prescription Coverage Creditable for Part D?
Employers providing prescription drug coverage to Medicare Part D-eligible employees and dependents must notify them if the coverage is creditable, as per Medicare Part D requirements.
- Notices should be given to Medicare-eligible active employees and dependents, COBRA individuals, disabled individuals, and retirees under the plan.
- Employer groups required to supply the notices to their participants include, but are not limited to private employers, governments, collectively bargained plans, and church plans.
- Since you may not know which employees have Medicare-eligible dependents, we recommend sending the notice to all employees.
We’ve provided a model notice for your convenience. You will need to fill in your group name, health insurance company, and contact information where indicated. The Model language for these notices was obtained from the Centers for Medicare & Medicaid Services (CMS) website.

Part D Changes and Creditable Group Coverage
Medicare Part D changes in 2025 may impact the creditable coverage status of some Group Health Plans. We created a video for your employees to review the options they have should their drug plan become non-creditable.
